Mosaic was established in July of 1993 as an affiliated program of an 85 year old Nebraska non-profit organization. We serve 2,100 children and adults in 12 states, Latvia and the U.K. Provo serves as the headquarters for the Utah program, which includes Nephi, Richfield and outlying areas.

Mosaic Utah benefits approximately 56 people in residential and vocational settings. Specialized programs ensure that persons served can progress to their fullest potential and live as independently as possible. Accredited with The Council on Quality and Leadership in Supports for People with Disabilities, the national accreditation agency in the field, Mosaic has received a three-year accreditation, with distinction, the highest rating the council awards.

Residential services consist of staff-assisted homes and supported living apartments in neighborhood settings, to promote independence, and to integrate people into the community as well as fostering the opportunity for life long friends. The vocational services are developed according to individual needs, choices, and abilities, and include supported employment at community businesses, to learn work skills in a supervised environment.

We are also licensed by the Utah Department of Human services, a qualified provider through the Utah State Office of Rehabilitation and a contracted service provider for the Division of Services for People with Disabilities.
