UACS Membership Application

This Membership Agreement is made and entered into by the Utah Association of Community Services (UACS) and the undersigned applicant. UACS is a statewide association representing disability service providers committed to excellence in service, advocacy, and the well-being of individuals with disabilities. Membership applications are reviewed and voted on by the Executive Council before a full membership vote at our monthly general membership meeting. Membership application have a $750 processing fee that is nonrefundable. Please complete the application currently and fully.
